Role Summary:
As a Practice Manager, you will report to a Team Manager and take on responsibility for managing and supervising a team of qualified and support staff. You will also support colleagues in delivering exceptional care and ensuring legal and regulatory compliance.
Your responsibilities will include:
- Managing a complex caseload and triages.
- Leading as a Duty Lead within the team.
- Developing person-led support plans for adults in Mental Health and Wellness.
- Working with vulnerable adults and older people, ensuring their safety, independence, and well-being.
- Coordinating with external partners and agencies to deliver integrated care services.
- Representing the Council at multi-agency meetings.
- Managing a team of staff, offering guidance and supervision to ensure high performance.
- Undertake the full legal range of social care assessments and develop person-led support plans.
- Maintain good working relationships with professional colleagues, external agencies, and service users.
- Demonstrate a high level of communication, negotiation, and assertiveness skills.
- Handle challenging situations, ensuring the safety and well-being of adults and older people.
- Provide direct line management and supervision for staff members.
Skills and Experience Required:
- Significant post-qualifying social work experience in a children and families or mental health setting.
- Experience of complex case management and risk assessment.
- Ability to make decisions based on thorough analysis and evidence.
- Demonstrable knowledge of working with multi-cultural communities and delivering culturally sensitive services.
- ICT proficiency, including Microsoft applications.
- Proven experience managing complex and challenging situations.
Qualification:
- Social Work Degree (BA or MA in Social Work).
- HCPC Registration as a qualified social worker.
- AMHP (Approved Mental Health Professional) Qualification.
- Post-qualifying experience in a relevant social care or mental health setting.
- Enhanced DBS Check (Disclosure and Barring Service).
